" Music For Life " 收」或交回任何一間通利琴行即可。申請一經批核，會員咭於四星期後寄上。
Please send the completed application form, together with a photocopy of all purchase receipts＊ ,
to Tom Lee Music Co Ltd, 11/F Silvercord Tower 1, 30 Canton Road, Tsimshatsui, Kowloon or by hand at
any of our stores. Please mark " Music For Life " on the envelope. Your membership card will be sent
to you by post after 4 weeks if your application is approved.
＊所有購物單據必須為申請日計起3個月或6個月（金咭申請）內發出方為有效。

＊All purchase receipts must be issued within 3 months from the date of application (6 months for Premier Gold application).
